Registering a death
Before you can register a death you must ensure that you have a Medical Certificate of Cause of Death.
Who can register a death?Any of the following people can register the death:
a) A relative or civil partner of the deceased
b) A person present at the death
c) The person responsible for the settling of the estate of the deceased
Under normal circumstances registration should take place within five days of the death.
Where should I register the death?The death must be registered in the borough that the person died
DUDLEY BOROUGH: You must make an appointment by contacting 0300 555 2345. You can register at either the Dudley or Stourbridge office.

What information will the Registrar require?
DOCUMENTATION you will need to give to the Registrar:
a) Medical Certificate of Cause of Death
b) The medical card of the deceased (if available)
INFORMATION you will be asked for by the Registrar:
a) The date and place of death
b) The full name and surname (and the maiden name if the deceased was a woman who had married)
c) The date and place of birth
d) Occupation (and if the deceased was a married woman or a widow the name and occupation of her husband)
e) The usual address
f) Whether the deceased was in receipt of a state pension or allowance
g) If the deceased was married, the date of birth of the surviving widow or widower
h) Name and surname of the person registering the death, reason for registering and their usual address
What will I be given by the Registrar?
Following registration you will be given:
a) The Certificate for Burial or Cremation (green) - this is the form you should hand to the funeral arranger at A & A Walters.
b) A Certificate of Registration or Notification of Death. This certificate is needed in order to claim benefits from the Department for Work and Pensions.
c) Copies of entry in the register - these are usually required for legal purposes and can be obtained on payment of a fee to the registrar.
